Liability and Media Release *

I understand and agree that neither the event nor its owners, operators, agents, or instructors, including but not limited to the National Adaptive Fitness Foundation, and/or The Chase Foundation, may be held liable in any way for any occurrence in connection with my (or whoever is being released) participation and performance, which may result in injury, death, or damages to me or my family, heirs, or assignees. I further acknowledge and forever release the National Adaptive Fitness Foundation, and/or the Chase Foundation in connection directly or indirectly with my participation and performance. Media Release: I grant permission to the National Adaptive Fitness Foundation, hereinafter known as the “Media” to use my image (photographs and/or video) for use in Media publications including: Videos, Email Blasts, Recruiting Brochures, Newsletters, Magazines, General Publications, Website and/or Affiliates. I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image. MUST BE 21 OR OLDER TO RECEIVE ALCOHOLIC BEVERAGE TICKETS
